National Readmission Prevention Collaborative

Mission

The National Readmission Prevention Collaborative was created to unite industry leaders in sharing Best Practices in care transitions and readmission prevention for hospitals, SNF's and other providers. NRPC provides educational resources, certifications, case studies, intensives and events as well as access to products, tools and services that improve the care continuum.

Vision

To showcase and share state-of-the-art 'best practices' and solutions in improving integrated care and readmission prevention to improve the care continuum nationwide.

Volunteer Organization

NRPC is a volunteer collaborative. NRPC is not a business entity nor is it a consultancy (it is neither a for-profit, nor a not-for-profit as both are IRS business entities). NRPC is a volunteer committee that meets four times annually (including via conference call). The main agenda items at the meetings of the volunteer collaborative include;

  1. Enhancing the website
  2. Hosting at least one annual conference
  3. Sharing new, innovative and advanced concepts in reducing unnecessary readmissions

Individual Advisory Panel members are volunteers. Further, the thoughts, opinions, examples, comments, stories, data, materials and case studies shared on this website by Advisory Panel members do not represent the opinion of their respective employers, but are the opinion of those individuals offering them. Information posted on this website is for informational purposes only.

When using the NRPC Site, information will be transmitted over a medium that may be beyond the control and jurisdiction of NRPC, Advisory Panel Members and its suppliers. Accordingly, NRPC assumes no liability for or relating to the delay, failure, interruption, or corruption of any data or other information transmitted in connection with use of the NRPC Site.

  • The accuracy, reliability, completeness, correctness, or timeliness of the Content, software, text, graphics, links, or communications provided on or through the use of the NRPC Site or NRPC.
  • The satisfaction of any government regulations requiring disclosure of information on prescription drug products or the approval or compliance of any software tools with regard to the Content contained on the NRPC Site.

In no event shall NRPC, Advisory Panel Members, its licensors, its suppliers, or any third parties mentioned on the NRPC Site be liable for any damages (including, without limitation, incidental and consequential damages, personal injury/wrongful death, lost profits, or damages resulting from lost data or business interruption) resulting from the use of or inability to use the NRPC Site or the Content, whether based on warranty, contract, tort, or any other legal theory, and whether or not NRPC, Advisory Panel Members, its licensors, its suppliers, or any third parties mentioned on the NRPC Site are advised of the possibility of such damages.

NRPC, its licensors, Advisory Panel Members, its suppliers, or any third parties mentioned on the NRPC Site shall be liable only to the extent of actual damages incurred by you, not to exceed U.S. $1,000.00. NRPC, Advisory Panel Members, its licensors, its suppliers, or any third parties mentioned on the NRPC Site are not liable for any personal injury, including death, caused by your use or misuse of the Site, Content, or Public Areas (as defined below).

Any claims arising in connection with your use of the Site, any Content, or the Public Areas must be brought within one (1) year of the date of the event giving rise to such action occurred. Remedies under these Terms and Conditions are exclusive and are limited to those expressly provided for in these Terms and Conditions.
